providing 24-hour comfort and care to adults
with life-limiting illness, and their families.
Our Adult Day Program is located at 192 Carlton Street. The Monday to Friday, all-day program is set in a cozy Victorian home in the heart of Cabbagetown.
This program is designed for the frail older adult, who requires specialized programs, small group activities, nutritious meals and transportation. It includes specialized programs for members with Dementia and Alzheimer’s and also offers caregiver respite and support.
Individuals who are 55 years of age and older and individuals living with physical disabilities, who are over 18 years of age. Applicants will be assessed prior to admission to this program and should live in the area bounded by Bathurst Street, Don Valley Parkway, St. Clair Avenue West and Lake Ontario.
